Advice & Assessment Service

If you think that you may require care in your home but are unsure of what sort of care you may need we can provide a Health Care Assessor to review, evaluate and advise as to your personal risks, needs and safety requirements.

This can include case co-ordination, decision-making, arranging appropriate care packages, providing or ensuring the provision of case management support and monitoring and reviewing the needs of individuals. As part of the wider process of assessing or reviewing an individual’s needs we can also give advice regarding Continuing Health Care eligibility.
